All infusion referrals are accepted by our Infusion Admissions team during normal business hours: Monday through Friday, 8:30 AM–5:00 PM. Any referrals sent outside normal business hours will be addressed the next business day.

If you call in referrals outside of normal business hours, our on-call staff will be able to assist you.

Here is what we need:

1. Patient Demographics & Insurance information

2. Diagnosis

3. H&P, 2-3 Progress Notes, Patient’s Height & Weight, Allergies, Labs and/or tests to support the diagnosis

4. Scripts for drug(s), flushes, and any necessary pre-medications. Anaphylactic orders for 1st dose.
